EXHIBIT A <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12 - <br />month contract period = _200 PERSONS <br />II. Description of Work - in space below, describe the program to be funded during the 12 -month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />According to the city clerk, the current population of Santa Ana is <br />approximately 318,000. Based on the widely accepted assumption that fifteen <br />percent of all individuals living in the United States are disabled, there are at <br />least 47,700 such individuals residing in the city. This number represents <br />approximately eleven percent of the county's total disability population. <br />The U.S. Department of Housing and Urban Development has <br />designated persons with disabilities as automatically eligible for assistance <br />under the low/moderate income classification of the Community Development <br />Block Grant Program. In fact, most members of this target group fall within <br />the extremely low-income range. <br />For most people, life with a major disability is a constant challenge. <br />Routine necessities such as housing and employment become seemingly <br />insurmountable obstacles, when coupled with unique requirements such as <br />wheelchair accessibility or sign language interpretation. Frequently, people <br />with disabilities and their families are overcome with a sense of helplessness <br />and hopelessness, because they are unable to obtain needed support. <br />During the tenth and twentieth years of operation, the Dayle McIntosh <br />Center conducted a countywide needs assessment to determine if independent <br />living services continued to be necessary among area residents with <br />disabilities. The findings of both these studies were similar, i.e. the need for <br />assistance in obtaining affordable housing, accessible transportation, adequate <br />health care, employment, and information regarding community resources and <br />disability -related topics. Many area residents fail to receive the support they <br />need either because they are unaware of the assistance available through the <br />Center or because they are reluctant to approach an organization that is out of <br />their immediate vicinity. This situation has prompted the Center to adapt its <br />service delivery methodology to allow for more direct outreach. <br />Goal: To increase the availability of independent living services for residents of <br />Santa Ana, who are significantly disabled. <br />EXHIBIT A <br />
