Objective 1: Implementation of a service delivery plan specifically for the <br />target area. <br />Activities: <br />• Assign a field service coordinator to work in Santa Ana two days per week; <br />• Provide laptop computer and cell phone for coordinator so that information <br />and resources will be available and so that consumer files might be <br />maintained; <br />• Research, compile, and update community resources within the area, which <br />may be used to assist clients; <br />• Maintain at least two satellite sites, where coordinator might meet with <br />local residents, who are disabled. <br />Objective 2: Initiation of outreach strategies to identify those, who might <br />benefit from independent living services. <br />Activities: <br />• Identify and contact all relevant public and private agencies in the city to <br />establish cross -referral procedures; 26 contacts annually; <br />• Conduct presentations at community sites and groups; 12 annually; <br />• Post notices on bulletin boards in public areas; 3 monthly; <br />• Place announcements in local newspapers and on local radio stations; 6 <br />annually; <br />• Attend community events within Santa Ana; 9 annually. <br />Objective 3: Provision of information, services, and training for Santa Ana <br />residents with disabilities. <br />Activities: <br />• Conduct intake procedure and open consumer files with new clients; 10 <br />monthly; <br />• Assess needs of incoming clients; 11 monthly; <br />• Develop service goals, objectives, and time -lines for persons seeking <br />specific assistance; 9 monthly; <br />• Coordinate services to assure that participating Santa Ana residents <br />receive necessary assistance; 100 clients annually; <br />• Achieve at least 80% of the goals, objectives, and time -lines defined in <br />the individual service plans of participants. <br />
