<br />. <br /> <br />. <br /> <br />EXHIBIT B <br /> <br />STATEMENT OF WORK <br /> <br />Scope of Work. City hereby engages contractor as an independent contractor to <br />perform the following described work, and contractor hereby agrees to perform <br />said work upon the terms and conditions here-in-after set forth. <br /> <br />A. <br /> <br />Specifically, contractor shall perform the following services: <br /> <br />Contractor shall provide services based on the four elements noted <br />below for each participant referred by the Employment <br />Development of the State of California (EDD). Chosen elements <br />will be noted in each participant's Individual Service Strategy (ISS). <br /> <br />1. <br /> <br />2. <br /> <br />4. <br /> <br />The four elements are: <br /> <br />a. <br /> <br />Tutoring, study skills and instruction leading to completion of <br />secondary school, including dropout prevention strategies. <br />Alternative secondary school services, as appropriate. <br />Occupational skills training as appropriate. <br />Leadership development opportunities, which may include <br />community service and peer-centered activities encouraging <br />responsibility and other positive social behaviors during non- <br />school hours as appropriate. <br /> <br />b. <br />c. <br />d. <br /> <br />Contractor shall establish at least one goal for each participant as <br />related to the elements identified in their ISS. If the participant is <br />deficient in basic reading and/or math skills, one primary goal must <br />be for basic skills. Goals are to be based on appropriate age <br />standards and entered in participant's ISS. <br /> <br />Contractor shall provide an enrollment orientation for each <br />participant which at the minimum includes the following information: <br /> <br />a. <br />b. <br />c. <br />d. <br /> <br />Program goals and objectives <br />Length of training period <br />Civil rights and complaint handling procedures <br />Available supportive services <br />Anticipated termination date <br />Job development activities <br /> <br />e. <br />f. <br /> <br />Participant orientation shall be documented and placed in <br />participant's file. An orientation of the Santa Ana W /O/RlK Center <br />will also be provided. <br />
