<br />... <br /> <br />. <br /> <br />. <br /> <br />Advice and Check Sheet for Amendment to EZ <br /> <br />To place funds under the Grant Agreement quickly so the funds are available for <br />use it is suggested that the below procedures be followed: <br /> <br />1. Make sure all signatures and dates and titles are filled in before forwarding to <br />governor's office. All parties who signed the Grant Agreement must sign the <br />amendment. See first paragraph of existing Grant Agreement if there are <br />questions. <br /> <br />2. Call Governor's Office when ready to mail to determine who will be handling the <br />package there. Please provide me with name/phone number and fax # of person <br />who is handling for the governor's office. <br /> <br />3. When package is received by coordinator in City/County, please call me to let <br />me know that it has been received-Thomas Terrell-202- 708-0614-x4673. <br /> <br />4. Please forward to city/county by hand if possible to expedite processing and or <br />use special messenger service to make sure expeditious handling occurs and <br />signatures, dates and titles are obtained quickly. <br /> <br />5. Use control mail/Fed Express, etc. to expedite signature of governor or <br />authorized state official and forward to Dennis Kane at HUD in Washington, <br />D.c. 20410-451 7th Street, SW Room 7130 when all signatures have been <br />obtained. <br /> <br />6. When Governor's office dispatches to Dennis Kane, please call Thomas Terrell <br />at 202-708-0614-x4673 to let him know that all three signed originals of the <br />amendment are on their way to Dennis Kane and the form of transmittal(Fed <br />Express). Dennis Kane's phone number is 202-708-6339 and title is EZEC <br />Coordinator. <br /> <br />7. Please place a copy of this Advice and Check Sheet in the transmittal package to <br />to the Governor's Office <br />
