County of San Joaquin <br />10 Proposal Format <br />All proposals are expected to follow this format. The purpose of this section is to ensure <br />the submission of the proposal information essential to the understanding and evaluation <br />of proposer's proposal. There is no intent to limit the contents of proposer proposals. <br />Any additional information, which a proposer deems appropriate, should be included and <br />submitted in an Appendix to the proposal. Proposals that differ from this format in a <br />manner that inhibits evaluation of the proposed solution may be rejected. <br />The proposal must include a "Master Copy" and six (6) All cost information shall be <br />submitted in sealed and separate envelopes labeled "PRICING FORMS ". <br />The proposal must include an electronic copy of all materials required in response to <br />templates provided with this RFP. All electronic copies of the cost forms shall be <br />submitted in a sealed and separate envelope labeled "ELECTRONIC PRICING <br />FORMS ". <br />10.1 Front Matter <br />An individual who is authorized to bind the proposer contractually shall sign a cover <br />letter, which shall be considered an integral part of the proposal. The signature must <br />indicate the title or position that the individual holds in the firm. In the case of solutions <br />provided by a partnership, the cover letter must clearly identify the prime contractor and <br />state that the prime contractor assumes all responsibility for the delivery of proposed <br />goods and services. An unsigned proposal will be rejected. <br />10.2 Section 1 - Executive Summary <br />The Executive Summary should not exceed three (3) pages and should present the <br />Proposer's understanding of what is to be provided to Counties and Cities as a result of <br />this contract, a brief background and History of your company, and at least three client <br />references, including name, organization, phone number, and email address. The <br />Statement of Work, the problems being addressed, and objectives. It should summarize <br />how the Proposer's proposal meets the Request for Proposal requirements and why the <br />Proposer is the best qualified. <br />10.3 Section 2: Statement of Conformance <br />In the second section of your proposal, please state the degree to which your proposed <br />solution meets the County's requirements and legal considerations. Should your <br />proposed solution vary in any way from these stated requirements, please identify those <br />areas in which your proposed solution differs and a brief discussion of the rationale <br />behind such differences. <br />15 <br />
