JUN -1-2004 01:52 FROM:K RUTLAND / CASA DE 7145713956 TO:6476�49 <br />Community Development Block Grant <br />Revised Scope of Work Prepared for the City of Santa Ana <br />CASA DE SALUD FAMILY HEALTH CLINIC <br />COMMUNITY DEVELOPMENT BLOCK GRANT <br />REVISED SCOPE OF WORK <br />PROGRAM YEAR 2004-2005 <br />PROJECT ABSTRACT <br />CASA de SALUD Family Health clinic has been a leader in the treatment of diabetes since the <br />clinic was founded in 2000. Specifically, CASA provides outreach, education, screening <br />intervention, and treatment services to a population at -high risk for developing diabetes ano its <br />related complications. These services are provided to a population with little or no access u <br />comprehensive health care services. Funding this project, Providing Our Diabetics <br />Empowerment Resources (PODER), will ensure CASA's continued provision of these muc) <br />needed services. Diabetics and those at risk for the disease and its related complications are <br />educated about and empowered to manage their disease. Education will be provided by tnv, <br />bilingual Promatores (Community Health Outreach Worker) who have special training anc. <br />experience in teaching about diabetes. In this context patients will learn about advancements ir <br />diabetes treatment with major emphasis upon preventing debilitating and life threatening <br />complications and developing the skills that empower individuals to modify lifestyle and manage <br />their disease. <br />GOALS AND OBJECTIVES <br />r <br />Drawing on proven self-management and preventive applications, CASA seeks to ensure that <br />low-income uninsured clinic users who have diabetes and are at risk of developing disease related <br />complications are equipped to nage their disease. <br />OutCOWeaAl <br />rtdtc�tors Objectives — <br />Better self-management.
