The internal evaluation will consist of three components <br />1. Individual patient progress will be closely monitored and will become part of a larger <br />database measuring program effectiveness. Lab results, including HgbA1C, will be <br />monitored. The ability to track patients' progress along with their treatment plan v ril <br />allow providers to closely analyze which variables lead to success and which are barriers <br />Trends will become readily apparent and will increase the knowledge base of hor't <br />providers and patients <br />2. Numbers of patients provided education will be measured. <br />3. We will also use random sampling techniques to track patient satisfaction with the <br />program over time. <br />E <br />
