<br />D. Outcomes <br />i. Eight-five (85%) of youth will receive one or more of the <br />following: <br />1. One Stop Center orientation which will include, but not be <br />limited to: an overview of program activities, job strategies <br />& techniques, and a presentation of available services; <br />2. Enrollment in a Certified Educational Program (ROP, <br />Vocational School, Alternative School); <br />3. Return to school; <br />4. Transition to Workforce Investment Act (WIA) programs; <br />5. Entry into unsubsidized employment <br /> <br />ii. Ninety percent (90%) of participants will attend a minimum of 6-8 <br />hours ofthe following workshops: <br />1. Employment Readiness Skills <br />2. Child Labor Laws <br />3. Post-Secondary Education infonnation <br />4. Life Skills <br /> <br />V. <br /> <br />WORK CENTER RESPONSIBILITIES: <br />A. Participant Recruitment/Orientation: Present a program orientation of all <br />available services. Infonnation will be collected from participants to <br />support program eligibility. <br />B. Work-site Development: Develop work sites by contacting local <br />businesses, public/non-profit and for profit agencies. Work-site <br />supervisors will be given a copy of the Work-site Supervisor Handbook <br />and answer questions about the program. <br />C. Each organization serving as a work-site will complete and sign a "Work- <br />site Agreement Form/Training Agreement Fonn" which, at a minimum, <br />will include a brief description of each participant's position, the assigned <br />hours and the tasks to be learned. <br />D. Maintain a file for each participant: eligibility documents, <br />registration/enrollment/exit fonns; Objective Assessment and Individual <br />Service Plan; EEO/Grievance; Participant and Work-site monitoring <br />fonns; Time card; Parental pennission fonns for minors in the program; <br />E. Participant InterviewsIMonitoring: Advise participants about potential <br />work-sites, schedule interview appointments for participants with letter of <br />introduction to work-site supervisors. <br />F. Work-site monitoring: one fonnal work-site monitoring is required; <br />G. Work Related Injuries: infonn all organizations serving as work-sites to <br />refer all work related injuries to the Work-site Supervisor Handbook for <br />the procedures for handling work-site injuries. <br /> <br />EXHffiIT A <br />
