Medical Premium Contributions: Effective January 1, 2005 = Maintain <br />Kaiser California CalPERS HMO rates, during the term of the agreement. <br /> <br />4. Dental Premium Contributions: Jul y 1, 2004 = $70/month ( current rate) <br /> July 1, 2005 = $70/month <br /> January 1, 2006 = $80/month <br /> January 1, 2007 = Reopener <br /> January 1, 2008 = Reopener <br /> <br />25F-1 <br />
