Application Cover Sheet <br />. Project Narrative and Budget W orksheet* <br />. Grant Assurances and Goveming Body Resolution <br />NOTE: The five urban areas that participated in FY03 UASI Part II <br />need not file new Assurance and Resolution forms if the ones on file with <br />OES are current. <br />. Funding Distribution by Jurisdiction <br />. Additional items for urban areas: <br />- core city and core county POCs; <br />- definition of the urban area; and, <br />- UA WG member information. <br />.
