<br />Part Three - Application Requirements and Process, Continued <br /> <br />Governing <br />Body <br />Resolution <br /> <br />Grant <br />Assurances <br /> <br />Funding <br />Distribution by <br />Jurisdiction <br /> <br />Application <br />Due Date <br /> <br />Application <br />Assistance <br /> <br />The Governing Body Resolution appoints agents authorized to execute any <br />actions necessary under this grant. All six transit agencies and the five new <br />urban areas must submit a Resolution. The Governing Body Resolution does <br />not have to be submitted with the application, however, no payments can be <br />made until a valid Resolution has been received by OES. <br /> <br />NOTE: The five urban areas that participated in FY03 UASI Part II need not <br />file a new Resolution form so long as the one on file with OES is current. <br /> <br />The Grant Assurances form lists the requirements to which each core city and <br />transit agency will be held accountable. All six transit agencies and the five <br />new urban areas must submit an Assurances form. The Grant Assurances do <br />not have to be submitted with the application, however, no payments can be <br />made until the Grant Assurances have been received by OES. <br /> <br />NOTE: The five urban areas that participated in FY03 UASI Part II need not <br />file a new Assurances form so long as the one on file with OES is current. <br /> <br />Grant recipients must identify funding distribution by jurisdiction. Each core <br />city and transit agency must indicate the amount of funds distributed !Tom <br />their allocation to other jursidictions. <br /> <br />Applications are due to the state by April 24, 2004. The hard copy documents <br />should be mailed to the address previously shown. The EXCEL spreadsheet <br />can be sent on a CD with the other hard copy documents, or it can be e- <br />mailed to Grant.Managementía1oes.ca.gov. Incomplete applications will not <br />be approved. <br /> <br />The state will work closely with the core cities and transit agencies <br />throughout the application process to ensure that the application meets grant <br />requirements.
