4. Technical Description/Proposal showing products proposed and implementation <br />methodology. Schematic diagrams and layout plan are recommended in the bid <br />package. <br />5. In a separate envelope, Lump Sum Price of the bid price. <br />RFP Documents: <br />Copies of the RFP documents can be requested from the City's Purchasing Division 714 <br />647-5460. <br />Required Forms: <br />The proposal shall include the following documents: <br />1. Proposal <br />2. Bid Cost <br />3. US Department of Justice — Office of Justice Programs From 4061/6 (3-91) <br />Proposal Due Date: <br />Proposals and the fee schedule are due 5:00 pm on Friday May Oh 2005. <br />Proposals shall be mailed or delivered to 20 Civic Center Plaza, Purchasing Division <br />Office, 4"' Floor, Room 429, Santa Ana, CA 92702, and Attention Terri Okazaki. <br />Inquires: <br />Questions regarding the proposal should be directed to Nabil Saba at 714 647-3378 <br />Insurance Requirements: <br />As a condition of this project to be awarded, bidding entity shall obtain and maintain <br />throughout the term of the project insurance as described below. A certificate of <br />insurance, naming the City of Santa Ana as an additional insured, shall be provided to the <br />City before the award. <br />❑ General Liability- $1,000,000 each occurrence <br />❑ Workers Compensation — Within limits required by the State of California <br />❑ Automobile Insurance - $1,000,000 combined single limit <br />The City reserves the right to reject all proposals. Proposals shall be enclosed in a sealed <br />envelope plainly marked "Sealed Proposal FOR RFP 05-035 DO NOT OPEN WITH <br />REGULAR MAIL" <br />
