<br />REVISED SCOPE OF WORK <br />City of Santa Ana <br />Household Hazardous Waste Program <br /> <br />Curbside proposes to accomplish the following tasks for the City of Santa Ana to comply <br />with the Household Hazardous Waste Grant provided to the City by the California <br />Integrated Waste Management Board. <br /> <br />Curbside will collect typical household hazardous waste from homes on an appointment <br />basis during the term of the grant in neighborhoods throughout the City. A minimum of <br />one vehicle and two staff members will be present for each collection. Advertising of the <br />program will be developed by Curbside and distributed by the City. A listing of <br />acceptable and unacceptable wastes is attached to this scope of work. <br /> <br />From March 12, 2005 through April 7, 2005 the fee for collection shall be $3300 per day. <br />From April 8, 2005 through May 27,2005 the fee for collection shall be $105.00 per stop <br />with a minimum of 10 stops. Beginning May 28,2005 and continuing through the term of <br />the agreement, the fee for collection shall be $105 per stop with no minimum. The fee <br />shall reimburse Curbside for the following services: <br /> <br />1. Develop and print brochures or flyers to promote and advertise program for <br />distribution twice in City's municipal service billing cycle. <br />2. Accept calls from residents and set up appointments <br />3. Provide residents with containers and instructions for setting out household <br />hazardous waste. <br />4. Provide door to door pickup for residents, homebound and elderly residents. <br />5. Package the waste for transport <br />6. Recycle the waste <br />7. Deliver a report at the end of each month for each event held during the month <br />8. Prepare a monthly evaluation <br /> <br />Excluded from the above fee is commercial waste, any waste items in containers of <br />greater than five gallons, business waste, unacceptable waste, unusually large <br />quantities of waste and waste from homes not in the generally designated area. <br /> <br />Residents with 55 gallon drums of used oil can pay Curbside to remove the waste. <br />Residents with business waste can pay Curbside to accept their waste. <br /> <br />TV's and computer monitors will be accepted at no additional charge to the residents of <br />the City. <br /> <br />Household hazardous waste will be collected and managed in accordance with State <br />and Federal law. A receipt will be prepared for each participant. The City will receive a <br />copy of each receipt. Waste will be transported to Curbside's facility in Anaheim and <br />shipped from there on manifests and/or bills of lading to facilities permitted to process <br />the waste. <br /> <br />Exhibit A <br />
