Service Number <br />AHFS shall provide and pay for atoll-free telephone number at AHFS's <br />place of business for use by City personnel to telephone from any location <br />where vending machines aze located to request maintenance service by <br />AHFS. This toll-free number shall be provided within thirty (30) days of <br />effective date of the resulting Price Agreement. The toll-free number shall <br />be clearly posted on all vending machines. <br />Service Hours <br />AHFS shall provide maintenance service during all hours that City <br />facilities aze open. Response time for problem calls is a maximum of four <br />(4) to eight (8) hours from time calls have been placed. Maximum <br />response time for emereenc~ is one (1) hour from time call has been <br />placed. <br />Collection of Monev <br />AHFS shall provide for the collection of all money deposited within each <br />vending machine at regulaz intervals during each location's hours of <br />operation. <br />Refunds <br />AHFS shall refund to personnel who lost money due to machine's <br />malfunction. For thispurpose, AHFS shall maintain a fund at each <br />location in the amount of $5.00 in nickels and dimes. <br />Malfunction Report <br />AHFS shall fiunish a malfunction incident report to the installation contact <br />person upon completion of each maintenance call. The report shall <br />include as a minimum the following: <br />1. Date and time notified <br />2. Date and time of arrival <br />3. Type of Machine <br />4. Description of Malfunction <br />5. Remedy <br />
