<br />homeless (and at-risk) individuals reside in the region. During the 2004-2005 report <br />period, Santa Ana continued to support numerous activities to meet the needs of its <br />homeless population. These activities are discussed in detail in the Continuum of <br />Care narrative below. Finally, the City did not undertake any activities that resulted <br />in involuntary displacement of households. <br /> <br />5. EFFORTS TO ADDRESS HOUSING NEEDS OF PERSONS WITH DISABILITIES: The housing <br />needs of individuals with disabilities were also addressed through various means <br />during the report period. <br /> <br />· The City's housing rehabilitation programs are designed to respond to the needs <br />of individuals with disabilities, Wheelchair ramps, wider doors, grab bars, and <br />lower counter tops are examples of the property improvements often included in <br />rehabilitation projects for persons with disabilities or with limited mobility, Two <br />mobile homes rehabilitated during the report period included "accessibility" <br />improvements. <br /> <br />· As part of the rehabilitation of the Santiago Villas senior project, five units were <br />completed to ensure the project complied with federal and state regulations <br />regarding handicapped accessible units. <br /> <br />· The Dayle Mcintosh Center for the Disabled provided employment supportive <br />services to 54 disabled Santa Ana residents during the report period with CDBG <br />funds. These services helped the disabled improve their economic status and <br />thus access to appropriate safe and decent housing. <br /> <br />· The Mental Health Association of Orange County assisted 323 mentally ill <br />homeless individuals with outreach and treatment services. Outreach included <br />referrals to appropriate housing. <br /> <br />· Housing services were also provided to persons with HIV/AIDS - a condition <br />often considered a disability or special need. Fifty-seven individuals were <br />assisted with long-term rental assistance, 84 with transitional housing, and 208 <br />individuals received short-term rental/utility assistance to prevent homelessness. <br /> <br />D. CONTINUUM OF CARE NARRATIVE <br /> <br />HUD has placed significant emphasis in meeting the needs of the homeless through a <br />Continuum of Care system. A Continuum of Care is a comprehensive and coordinated <br />housing and service delivery strategy that responds to the different needs of homeless <br />and at-risk individuals and families, and helps them attain self-sufficiency. As a <br />recipient of CDBG, HOME, ESG and HOPW A funds, Santa Ana supported numerous <br />public and nonprofit agencies that reach out to the homeless, bring them into the <br />system of care, and facilitate their movement toward self-sufficiency.
