Any upgrading, repair work, or any installation solicited by the City, shall be considered as <br />extraordinary. Any extraordinary maintenance reqnests from the City shall be confirmed by <br />the Contractor as to the time of and number of personnel dispatched. The Contractor shall <br />contact the Enterprise Management Section regarding any extraordinary maintenance work <br />found to be beyond requested repairs and seek prior approval before the work is scheduled. <br />Contractor shall notify Public Works Agency (PWA) Dispatch Office at 647-3380 each <br />day prior to crews being dispatched by Contractor. <br /> <br />I. No underground work shall be performed prior to completion of Underground Service <br />Alert (USA) markings. <br /> <br />2. An electrical permit shall be obtained from the City's Planning & Building Department <br />prior to any additions to the City's street light or other electrical system. <br /> <br />3. All extraordinary work completed by Contractor shall be invoiced time and material as <br />specified on Contractor's bid proposal. <br /> <br />C. Contractor shall respond to requests by City for extraordinary work within 24 hours of the <br />request. Work shall start in a timely manner. Contractor shall dispatch personnel, materials <br />and equipment on a daily basis until work is completed unless conditions do not allow work <br />to be performed. <br /> <br />D. City shall pay for such extra ordinary repairs required as follows: <br /> <br />I. Materials. The City shall pay to the Contractor for auy materials used in <br />extraordinary maintenance at Contractor's cost from the snpplier plus the <br />percentage specified in the Contractor's bid proposal. All materials and parts shall be <br />new or have the approval of the Enterprise Management Section if otherwise. The City <br />has the right to inspect the Contractor's records to verifY any material costs used in <br />extraordinary maintenance. The City may provide replacement poles and fixtures. <br /> <br />2. Direct Labor. Contractor shall furnish a monthly report of hours spent in extraordinary <br />maintenance of street light and appurtenances per address. City shall pay the Contractor <br />for such hours of extraordinary maintenance performed at the iob site address at the rates <br />specified on the Contractor's bid proposal. Crews shall be dispatched so as to maximize <br />work performed. Said hourly rates shall be the total cost to the City. Rates shall include <br />all compensation for wages, profit, overhead, any fringe benefits such as employer <br />payments to, or on behalf of worker for health and welfare, insurance, worker's <br />compensation, pension, vacation, sick leave or any local, State, Federal or union tax or <br />assessment. <br /> <br />3. Equipment. City shall pay Contractor for equipment used in extraordinary maintenance <br />on a per day basis rate as specified in the Contractor's bid proposal. Equipment shall be <br />dispatched so as to maximize work performed. <br /> <br />11 <br />
