This endorsement, effective 12:01 AM, February 28, 2007 <br />Forms a part of Policy No: EG2223762 <br />Issued to: Ware Disposal Company, Inc. <br />By; Select Programs Insurance Services <br />WHO IS AN INSURED <br />S. BROAD FORM NAMED INSURED <br />The following is added to SECTION R - WHO <br />IS AN INSURED, Paragraph 2., as <br />Subparagraph (e): <br />(e) Any subsidiary and subsidiary thereof. <br />of yours which is a legally incorporated <br />entity of which you own a financial <br />interest of more than 50% of the <br />voting stock on the effective date of <br />Coverage. The insurance afforded <br />herein for any subsidiary not listed as a <br />named insured does not apply to injury <br />or damage with respect to which such <br />an insured is also an insured under <br />another policy or would be an insured <br />under such policy but for its <br />termination or the exhaustion of its <br />limits of insurance. <br />S. NEWLY FORMED OR ACQUIRED <br />ORGANIZATIONS <br />SECTION li - WHO IS AN INSURED, <br />Paragraph 4., Subparagraph a. is deleted in its <br />entirety and replaced with the following: <br />a. Coverage under this provision is <br />afforded only until the 180th day after <br />you acquire or form the organization or <br />the end of the policy period, whichever <br />is earlier, provided that coverage of the <br />newly formed or acquired <br />organizations is not excluded either by <br />the provisions of the Coverage Part or <br />by endorsement; <br />10, ADDITIONAL INSUREDS BY CONTRACT. <br />AGREEMENT OR PERMIT <br />a. The following is added as Paragraph 6. to <br />SECTION It - WHO IS AN INSURED: <br />6. Any person or organization with whom <br />you agreed, because of a written <br />contract or agreement or permit, to <br />provide insurance such as is afforded <br />77479 (1101) <br />C11266 <br />under this Policy, but only with respect <br />to your operations, "your work" or <br />facilities owned or used by you <br />provided: <br />(1) The written contract or agreement <br />has been executed or permit has <br />been issued prior to the bodily <br />Injury, property damage or personal <br />and advertising injury; <br />(2) The person or organization is not <br />included as an insured under <br />Section 11. of this Endorsement; <br />or <br />(3) The person or organization is not <br />included as an insured by an <br />endorsement issued by us and <br />made a part of this Coverage Part. <br />b. When an engineer, architect or surveyor <br />becomes an insured under Section 10. of <br />this Endorsement, the following additional <br />exclusions apply: <br />Bodily injury, property damage or <br />personal and advertising injury arising <br />out of the rendering of or the failure to <br />render any professional services by or <br />for you, including: <br />(1) The preparing, approving, or failing <br />to prepare or approve maps, shop <br />drawings, opinions, reports, <br />surveys, field orders, change <br />orders, designs or specifications; <br />and <br />(2) Supervisory, inspection, or <br />engineering services. <br />c. When a lessor of leased equipment <br />becomes an insured under Section 9. of <br />this Endorsement, the following additional <br />exclusions apply; <br />(1) To any occurrence which takes place <br />after the equipment lease expires; or <br />PAGE 4 OF 11 <br />
