Referrals for comprehensive guidance and counseling for legal, medical, <br />substance abuse, family/personal, health, housing, etc. issues <br />ADULT MENTORING (occurs throughout program as staff works with <br />corpsmembers in various capacities) <br />TRANSITION SERVICES: 10-13 Weeks based on individual needs <br />1. Pre-employment skills training <br />2. Individualized job search and placement <br />3. Individualized support/assistance for continuing post-secondary <br />education; financial aide packets, enrolling on-line, etc. <br />4. Individualized case management and support services <br />5. Adult mentoring <br />PROGRAM COMPLETION: <br />30 day timeline for post -corps placement; Maybe extended based on individual <br />needs or circumstances <br />POST CORPS TRACKING: Unspecified time, available as long as former <br />corpsmembers seek help and support, minimum of 12 months <br />1. One on one follow up; monthly contact to monitor, update, and revise 12 <br />month plan as needed; <br />2. One on one employment search and support <br />3. One on one case management and support services <br />Illustrate hota the organization will interface with the Santa Ana IVORK Center. <br />The OCCC will interface with the WORK Center as follows: <br />• OCCC will provide recruitment information and materials for distribution at <br />the WORK Center; <br />• OCCC will actively participate in provider meetings and will provide regular <br />updates to WORK Center staff regazding program status, recruitment, and job <br />placements; <br />• OCCC will host cotpsmember interviews at the WORK center at least once a <br />week or more often as needed; <br />• Corpsmembers will tour the Santa Ana WORK Center to meet staff and become <br />familiar with WORK Center services; <br />• Corpsmembers in the Transition phase of the program will establish an <br />individual Caljobs account and will learn how to access services at the WORK <br />Center. <br />Page 15 of 22 <br />
