<br />Effective January 1, 2006: <br /> <br />A. One hundred percent (100%) of the premium cost for <br />"employee-only" coverage. <br /> <br />B. Up to ninety dollars ($90) per month per employee for <br />"family" coverage. <br /> <br />Effective January 1, 2007: <br /> <br />A. One hundred percent (100%) of the premium cost for <br />"employee only" coverage. <br /> <br />B. Up to one hundred dollars ($100) per month per employee <br />for "family" coverage. <br /> <br />Effective January 1, 2008: <br /> <br />A. One hundred percent (100%) of the premium cost for <br />"employee only" coverage. <br /> <br />B. Up to one hundred ten dollars ($110) per month per <br />employee for "family" coverage. <br /> <br />Any contribution necessary to maintain benefits under said <br />dental plans in excess of the amount set forth above shall <br />be borne entirely by the employee. <br /> <br />For each such employee who is covered under a spouse's non- <br />City sponsored dental plan, the City will pay the employee a <br />cash payment each month in an amount equal to one-hundred <br />percent (100%) of the City's contribution, if the employee <br />waives, in writing, City-paid coverage. Said waiver shall <br />include a provision warning such employee that reentry into <br />any City-sponsored plan is allowed only at open enrollment, <br />unless there is a qualifying event, and may require proof of <br />insurability for such employee and/or family. <br /> <br />12.3 Disability Insurance. The City shall continue to pay one <br />hundred percent (100%) of the premium cost for a long term <br />disability insurance plan for employees covered by this <br />Agreement. <br /> <br />12.4 Life Insurance. The City shall continue to pay one hundred <br />percent (100%) of the premium cost for term life insurance <br />coverage under the policy it maintains on behalf of its <br /> <br />47 <br />
