<br />ARTICLE XII <br /> <br />12.0 EMPLOYEE INSURANCE <br /> <br />12.1 Health Insurance. <br /> <br />A. The City shall contribute toward the payment of premiums <br />for affected employees and their dependents under the <br />CalPERS Health Program. Effective July 1, 2005, January <br />1, 2006, January 1, 2007, and January 1, 2008, <br />respectively, the City shall contribute toward medical <br />premiums an amount consistent with the rates then in <br />effect for the "employee only", "employee plus one", and <br />"family", tier respectively, of the Kaiser "Other <br />Southern California" CalPERS HMO plan. Effective January <br />1, 2006, the City shall establish a Cafeteria benefit <br />plan for employees covered by this Agreement. <br /> <br />B. Any contribution necessary to maintain benefits under any <br />health insurance program provided by the City for its <br />employees and their eligible dependents in excess of the <br />amounts of the City contribution specified above shall be <br />borne entirely by the employee. <br /> <br />C. For each such employee who is covered under a spouse's <br />non-City sponsored health plan, the City will pay the <br />employee a cash payment each month in an amount equal to <br />one-hundred percent (100%) of the monthly premium amount <br />for the City's lowest "employee-only" coverage, if said <br />employee waives, in writing, City-paid coverage. If an <br />employee waives City provided coverage, said employee <br />shall provide proof of medical insurance coverage in a <br />non-City sponsored health plan. Said waiver shall <br />include a provision warning such employee that reentry <br />into any of the City-sponsored plans is allowed only at <br />open enrollment, unless there is a qualifying event, and <br />may require proof of insurability for such employee <br />and/ or f amil y . <br /> <br />12.2 Dental Insurance. During the term of this Agreement, the <br />City agrees to contribute toward the payment of premiums for <br />dental insurance plans provided by the City for employees <br />covered by this Agreement and their eligible dependents: <br /> <br />46 <br />
