Get names and addresses of all witnesses and try to keep <br />them on the scene until police and/or a supervisor arrive <br />COMPLETELY fill out the accident report before leaving the location. <br />If drive -able, bring the cab immediately to the yard for photographs and to fill out the <br />collision report. <br />FAILURE TO REPORT ANY COLLISION WILL RESULT IN IMMEDIATE <br />TERMINATION AND FORFEI'T'URE OF YOUR DEPOSIT. <br />COLLISION DISCIPLINARY ACTION <br />1. Any collision not reported immediately to dispatch, will result in a fine of <br />$100 to the driver and possible termination of lease. <br />2. Any driver leaving the scene of the collision, without authorization from <br />dispatch will be fined $100. <br />3. Any driver involved in a collision and not making a report to the office of A <br />Taxi Cab in a timely manner will be fined $100. <br />4. Drivers must come to the office immediately upon being released from the <br />scene of the collision and will be placed out of service until he has completed <br />his collision report at the office. <br />5. Any driver involved in a collision will be subject to an alcohol and drug <br />screen. <br />6. Any driver involved in a collision may be subject to attend safety training or <br />driver education classes. <br />7. Road supervisor /investigator may recommend further driver suspension.
