No. 05-127) <br />February 21, 2006 <br />Page 2 <br />A Request For Proposals was issued and <br />2005, and proposals were solicited. <br />requested and received is as follows: <br />9 Request F <br />5 Proposals <br />Proposals <br />~r Proposal <br />received <br />advertised on December 9 and 12, <br />A summary of the proposals <br />mailed <br />received from Santa Ana vendor <br />Proposals were evaluated based on compliance to defibrillator features <br />in ease of use, reliability and technology, training offered, warranty, <br />cost of equipment and supplies, completion of questionnaire, and <br />verification of authorized dealership. Given the Fire Department's <br />interest in expanding the use of defibrillators throughout their <br />facilities, Zoll Medical Corporation extended its offer to include both <br />the defibrillators for the Jail and the Fire Department. <br />The proposal received from Emergency Medical Equipment, LLC, a Santa <br />Ana vendor and a Zoll Medical Corporation authorized dealer, received <br />the highest percentage in the scoring process including the lowest cost <br />offer, meets or exceeds the evaluation for the 3 Zoll AED <br />defibrillators for the Jail. The Zoll AED defibrillator units for the <br />Fire Department will be processed through the parent company of Zoll <br />Medical Corporation. Additionally, funds are included for the supplies <br />on the existing 10 defibrillators in use by the Fire Department. <br />FISCAL IMPACT <br />Funds are available in the Inmate Welfare Fund; the Fire Department <br />Metro Medical Response System 2004 Grant and Fire Suppressions/EMS <br />Operating Materials & Supplies accounts (account nos. 23-349-6391, 146- <br />325-6391 and 11-323-6391). <br />APPROVED AS TO FUNDS AND ACCOUNTS: <br />c~ Francisco Gutierrez <br />Executive Director <br />Finance & Mgmt.
