o Review participant files periodically to ensure completeness and accuracy; <br />o Maintain logs of all CHCADA staff visits service contacts; <br />o Maintain participants' hard copy files in alphanumeric sequence at La Familia's <br />administrative site for the program; <br />o Handle, store, protect and control documentation throughout the life cycle of the <br />program; and <br />o All other data maintenance duties as required. <br />Task 8: Collaborate with Youth Provider Nefwork <br />The Program Coordinator will maintain data on the number of participants, <br />entry and exit date from the program, services received, problems encountered, <br />problem resolution strategies, and on other areas. Participant data relevant to <br />services by other providers in the Youth Provider Network will be shared and, <br />when appropriate and necessary, meetings with other Network staff will be held. <br />Task 9: Timely Submission of Case and Activity/Service Data (Ongoing Case <br />Management) <br />All internal CHCADA service providers will provide weekly data on the <br />services and service units provided as a prerequisite to the receipt of paychecks. <br />Services provided by Fuerzas Unidas and other Network members are closely <br />tracked through the use of the WIA Virtual One-Stop (VOS) System. All pertinent <br />information regarding participant appointments scheduled (and/or missed), all <br />services provided, and all other case notes, will be directly entered into the VOS <br />system in a timely basis. This Case Management component will be provided <br />throughout the program as well as during the twelve-month follow-up period. <br />I.E. FACILITIES -Location, Hours of Operation, Safety <br />The majority of FUERZAS IJNIDAS' services are delivered at CHCADA's La <br />Familia Family Services location, 1905 N. College Avenue, in Santa Ana. <br />Services are available Monday through Saturday, 8:00 am to 7:00 pm. La <br />Familia's site consists of a group of mini-suites of offices out of which we provide <br />numerous services, including, but not limited to, family counseling services, child <br />monitored visitation, child care, court-ordered child exchanges, youth substance <br />abuse prevention programs, and others. The area is surrounded by a middle <br />school, a medical facility and a shopping center -all family-friendly and in a safe <br />neighborhood very close to public transportation. Thus, CHCADA's FUERZAS <br />tJNIDAS is indeed located in a safe and youth friendly environment. <br />31 <br />
