<br />'"" <br /> <br />'-' <br /> <br />V. <br /> <br />GOALS AND OBJECTIVES! <br />IMPLEMENTATION STRATEGY <br /> <br />In the space below, please list the primary goals and objectives for your program. Provide <br />projected numbers of Santa Ana residents to be served, and the kinds of services they would <br />receiv'e if your program is funded. Also describe the specific actions and"tasks that would be <br />performed on a day-to-day basis to achieve these goals. <br /> <br />Mercy House Center anticipates serving 525 clients through individualized services in the <br />coming year. The primary goals of Mercy House Center are as follows: <br /> <br />- To increase client's knowledge of and access to resources <br />o Obiective: to provide emergency case management services to clients <br />Task/Action-assess client needs on an individual basis and refer to <br />appropriate agencies such as recovery programs or mental health. <br />- To improve the living conditions of clients <br />o Obiective: to assist clients with their basic needs; hygiene, food, clothing, laundry <br />Task/Action-provide clients with a "basic survival package" containing <br />hygiene items, a clothing or food voucher, laundry soap, socks, <br />etc. <br />- To improve client's mobility so they may access resources and obtain employment <br />o Obiective: to provide transportation assistance <br />Task/Action-provide clients with bus tickets (OCTA or Greyhound) or gas <br />vouchers <br />-To improve client's physical health <br />o Obiective: to help with prescriptions, eyeglasses and over the counter <br />medications <br />Task/Action-in addition to existing agreement with local pharmacy, seek <br />local professionals willing to donate services such as eye exams at <br />low costs <br />-To assist clients in moving toward self-sufficiency <br />o Obiective: to improve jab readiness <br />Task/Action-assist clients in obtaining the necessary materials for jobs (I.e. <br />boots or gloves) Continue to post employment center info on the <br />Center's bCllletin board <br />- To improve the lives of the chronically homeless <br />o Obiective: to reduce loneliness and provide positive social interaction <br />Task/Action-€ncourage discussion among clients and staff at the Center <br />and plan events at the center that promote social skills <br /> <br />q{f1181T t\; <br />
