<br />. <br /> <br />Description of Outcome Tracking System: <br /> <br />Please describe your organization's outcome tracking system that will enable you to fulfill the outcome selected above. <br />The Foundation deals almost exclusively with individuals who are substance addicted and in the Drug Court recovery program or in <br />other courts - such as DUI Court and the Co-occurring Court that use the drug court recovery model. The Foundation will enable <br />some Santa Ana residents to enter, or remain in the Drug Court program through its treatment bed scholarship program and will <br />maintain statistics on the nwnber of people provided entry during the 2006-07 fiscal year. The Foundation will also provide a variety <br />of service that include linking substance addicted people to other services such as Women Helping Women and other programs, such as <br />Lenscrafters "Gift of Sight" program that provide services the Foundation is unable to provide. The Foundation will maintain statistics <br />as to the number of substance addicted persons to whom the Foundation provided services as well as the number of substance addicted <br />persons linked or referred to other public and private service programs. <br /> <br />Exhibit A-I <br />Page 3 of 4 <br />
