<br />Exhibit B <br />S1-V-07 <br />C. File Maintenance and Documentation: A case file shall be maintained for eyery <br />enrolled Veteran. At a minimum, the case file will include information and <br />documentation of each of the following: <br />1. Program eligibility/determination of need; <br />2. WIA MIS forms, including source documents for yalidation; <br />3. Initial and ComprehensiYe Assessments; <br />4. IEP, including all updates of services proyided and completed; <br />5. Approyed ITA youcher, if applicable; <br />6. Progress reports, time and attendance if receiYing WIA and non-WIA funded <br />training (including short-term pre-yocational training); <br />7. Counseling notes printed from the computer. <br /> <br />E. Confidentiality: Contractor agrees to the following: <br />1. That all applications and individual records related to services proyided under <br />this Agreement, including eligibility for services, enrollment, and referral shall be <br />confidential and shall not be open to examination for any purpose not directly <br />connected with the delivery of such services. That the sharing of indiyidual and <br />client information necessary for proYision of services under this Agreement; Le.: <br />assessment; uniyersal intake; program or training referral; job development, <br />placement or follow-up actiYities; and other services as needed for employment <br />or program support purposes, constitutes a yalid use of such information. <br /> <br />2. That files are kept in a secure location on site and should not be removed for <br />any reason unless OCWIB Contract Administrator is notified of removal. <br /> <br />3. That no person or employee shall otherwise publish or disclose, use, or permit, <br />cause to be published, disclosed or used, any confidential information <br />pertaining to One-Stop System applicants, participants, or customers oyerall. <br /> <br />4. To abide by the current confidentiality proYisions of respectiYe statutes and <br />share information necessary for the administration of programs operated <br />through the One-Stop System. <br /> <br />v. PERFORMANCE <br /> <br />A. Performance Measures: The Workforce Inyestment Act requires a comprehensive <br />accountability system to determine the effectiyeness of services proYided. The <br />Contractor agrees to meet or exceed required federal, state and local standards, <br />measurements and outcomes of all funding streams included in this agreement. At <br />a minimum, Contractor shall meet all performance measures outlined in Exhibit C. <br />Contractor's performance statistics shall be evaluated monthly and are expected to <br />meet or exceed the performance targets on a quarterly basis. <br />1. Performance on new participant enrollments will be assessed by comparing the <br />Contractor's planned versus actual year to date cumulatjye new numbers. <br />2. Performance on soft exits will be evaluated quarterly. <br />3. Performance on required follow-up will be measured on a quarterly basis. <br />4. Performance on the mandated measures will be evaluated quarterly. <br />5.
