0 <br />J <br />,ham <br />REQUEST FOR PROPOSAL <br />JEROME CENTER RENOVATION <br />1. GENERAL <br />The City is soliciting proposals from professional consulting firms to provide <br />architectural design services to prepare contract documents (plans, <br />specifications and "Opinions of Probable Cost") and to provide limited field <br />observation during the renovation of the Jerome Center. <br />Upon receiving this Request for Proposal (RFP), consultant shall notify the <br />City regarding its intent to respond to the RFP. <br />From the proposals received, the City will enter into an agreement with the <br />selected firm. <br />The consultant must be able to assist the City through this contract to provide <br />the services needed. The consultant shall utilize in-house staff and/or sub - <br />consultants satisfactorily to the City to complete the assignments. For <br />specialized work for which the prime consultant will require a sub - <br />consultant, the prime consultant will serve as an administrative liaison <br />between the City and the sub -consultant. The prime consultant mark-up <br />for sub -consultants shall not exceed 10%. <br />II. CONTENT OF PROPOSAL <br />In order to maintain a uniformity with all proposals furnished by Consultants, it <br />is hereby requested the proposals be limited to a maximum of 15 pages <br />(excluding front and back covers, section dividers and resumes) and includes <br />the following: <br />A. A project understanding statement containing any suggestions or special <br />concerns that the City should be made aware of, and a project approach <br />containing any additional scope of work tasks you feel are necessary for <br />the successful completion of the project. <br />B. A project team organization chart identifying those who will perform work <br />and a brief resume of each team member, including similar type projects <br />in which they have been directly involved. Identify the Project Manager <br />(PM) and Project Engineer proposed for this project. The PM will be the <br />primary contact person to represent your firm and will be the person to <br />conduct the presentation, if invited for an interview. Sub -consultants, if <br />any shall be identified with the same requirements as for the prime <br />consultant. <br />C. List of similar projects that your firm completed within the last 5 years. <br />Project information should include project description, agency or client <br />name along with the person to contact and the telephone number(s), year <br />completed and project construction and design cost. <br />4 <br />
