<br />Coordinator is available by telephone, e-mail and in person at any time should <br />the need arise. <br />Participant Flow - CHCADNLa Familia's FUERZAS UNIDAS' Coordinator will <br />ensure that each of the required provider responsibilities outlined in the RFP are <br />met or exceeded. Outlined in the subsections below are the tasks that will be <br />performed to accomplish this. <br /> <br />Task 1: Designate Program Coordinator/Supervisor <br />The Project Coordinator is a key component of La Familia's service <br /> <br />delivery mechanisms. This 80 percent FTE staff person will perform the duties <br /> <br />and activities outlined in Figure 5 below. <br />Figure 5: Duties and Responsibilities of Program Coordinator <br />~ Coordinate and schedule all services provided by CHCADA's staff <br />:>- Be phYSically present at each site when services are being provided <br />:>- Train CHCADA staff regarding policies, procedures and regulations at each site <br />:>- Participate in monthly meetings with La Familia's Project Director <br />" Participate In all required meetings at the Santa Ana W/O/RJK center, with WIA Youth <br />Network and other key entitles <br />:>- Remain informed regarding all youth In the program <br />:>- Intake other youth In collaboration with other Network members <br />" Observe youth and make case notes based upon the observational data as part of the <br />assessment process <br />:>- Provide input to CHCADA's service team regarding the service mix and intensity of services <br />required by each youth <br />:>- Make logistical arrangements associated with the provision of services to youth by other <br />Network members, i.e., transportation, service location, etc. <br />:>- Maintain inventories of matenals and items needed for service delivery <br />,. Serve as counselor; and <br />:>- Complete other duties as requested by CHCADA's Senior Management, La Familia's Project <br />Director and/or WIA contact person. <br /> <br />The Program Coordinator for this effort is Ms. Yvonne Elizondo. The <br /> <br />Program Coordinator's qualifications include each item listed in Figure 6 below. <br /> <br />Fi ure 6: Qualifications of Pro ram Coordinator/Su ervisor <br />v" An Associate's Degree in a Social services area; <br />v" Two or more years in a youth program; <br />v" One or more years of experience with at-risk youth; <br />v" Two or more years of experience as a youth counselor; <br />v" Bilingual/Bicultural in English and Spanish; and <br />v" Skilled in the use of Microsoft EXCEL and/or other data mana ement <br /> <br /> <br />Task 2: Outreach/Recruitment/Eligibility Determination/Intake/Assessment <br /> <br />18 <br />
