<br />As the service provider for this program, CHCADNLa Familia will perform <br />the following steps in the delivery of services to the youth participants: <br />Outreach/Recruitment - as previously described in detail in Section IC. <br />GOALS/OBJECTIVES AND PERFORMANCE LEVELS <br />o Eligibility Determination - once recruited, potential participants are then <br />screened to ensure that they meet the WIA eligibility requirements as set forth in <br />the RFP. Prospective participants who meet the requirements and are accepted <br />into the FUERZAS UNIDAS program are subsequently taken through an <br />Intake/Assessment process, as previously described, with the Coordinator <br />and/or Case Manager to determine the level of need so that an Individual <br />Service Plan is developed for each participant and services may be provided. <br />o Referrals - are provided to appropriate Resources as required (i.e., nutrition and <br />health, psychological counseling and other human resources). <br /> <br />Task 3: Implement Program <br />CHCADNLa Familia's program services include primary group <br />counseling, mentoring, tutoring, leadership development, support services, <br />employment readiness support intensive follow-up, and other services through <br />FUERZAS UNIDAS at our Santa Ana location. The general characteristics of our <br />program are as listed in Figure 7 below. <br /> <br />FiQure 7: Key Characteristics of FUERZAS UNIDAS <br /> <br />. CHCADA will schedule at least 300 minute-per-day for the youth to participate in a <br />training program through the Youth Network and/or an educational program (i.e., <br />Taller San Jose, aGee, etc.); <br /> <br />. CHCADA will utilize unique pedagogies that nevertheless reflect the program's <br />employment focus. For example, for some youth we may use an arts and drama <br />focus, i.e., the youth may be asked to stage a psychodrama that highlights how to <br />avoid gang involvement and/or prepare posters and artwork that reveals positive <br />aspects of their family relationships. Likewise, youth may be asked to write a short <br />story about the dangers of gang involvement. <br /> <br />CHCADA will meet the counseling and leadership development <br />requirements by providing two-hour group sessions/mini courses on key topics. <br />Each group session/mini course will have no more than 15 participants. <br />CHCADA's curriculum for the program includes thirteen (13) courses _ ten (10) <br />core courses and three (3) specialized courses that will have relevance for the <br />selected youth. Figure 8 below lists the group counseling/mini courses that are <br />offered. <br /> <br /> <br />19 <br />
