PERSONNEL DISCLOSURE <br />25.1 CONTRACTOR shall make available to ADMINISTRATOR a current list of <br />all personnel providing services hereunder, including resumes and job <br />app]ications. Changes to the list will be immediately provided to <br />ADMINISTRATOR in writing, along with a copy of a resume and/or job <br />application. The list shall include: <br />25.1.1 All full or part-time staff positions by title, including <br />volunteer positions whose direct services are required to provide the programs <br />described herein: <br />25.1.2 A brief description of the functions of each position and <br />the hours each person works each week, or for part-time personnel each day or <br />month. as appropriate; <br />25.1.3 The professional degree, if applicable, and experience <br />required for each position; and <br />25.1.4 The language skill, if applicable, for all personnel. <br />25.2 Where authorized by law, CONTRACTOR shall conduct criminal record <br />background checks on all employees and/or volunteers who will provide services <br />under this Agreement. <br />25.3 CONTRACTOR warrants that all persons employed or otherwise <br />assigned by CONTRACTOR to provide services under this Agreement have <br />satisfactory past work records and/or reference checks indicating their <br />ability to perform the required duties and accept the kind of responsibility ~, <br />(WBC0607) <br />32 of 40 <br />(April 9. 2007) <br />
