• City staff will provide a monthly work request identifying properties to be researched and <br />categorized. <br />• Windshield survey results and templates will be transmitted electronically in a Microsoft Word <br />format. <br />• City will be responsible for ownership, Assessor's Parcel Numbers, USGS Quad information, and <br />sketch maps as part of the DPR form. <br />• City will continue to provide building permit access. <br />• City will continue to provide copies of relevant documentation. <br />• City will provide a base map and comprehensive database of addresses and associated dates of <br />construction for assigned neighborhoods <br />• Up to seventy-two (72) templates will be prepared. <br />TASK 2. MEETINGS. <br />Sapphos' Architectural Historian will attend up to 18 meetings as follows: I (Historic Resources <br />Commission (and Template Committee meetings, as scheduled) on the first Thursday of each month, and 6 <br />coordination meetings with Staff. For budgeting purposes, an average of five (5) hours per meeting has <br />been assumed. <br />TASK 3. MISCELLANEOUS SPECIAL INTEREST TASKS. <br />From time to time, at the City's request, the Consultant will undertake additional tasks including assisting <br />the City with reconnaissance -level surveys and historic assessments, providing additional documentation <br />for the City to submit to Office of Historic Preservation (OHP), assisting City staff with the application of <br />the Secretary of the Interior's Standards, conducting additional site inspections. A budget for such tasks <br />has bee included in the current proposal. Sapphos will notify the City when and if this budget is depleted, <br />or if any tasks would request budget augmentation. <br />TASK 4. OTHER REQUIRED SERVICES. <br />Sapphos will prepare and submit a monthly status report, and will conduct periodic project reviews and <br />audits. <br />BUDGETITERMS AND CONDITIONS <br />Not to exceed fees are as follows: <br />Task I CATEGORIZATIONS $59,592.80 <br />Task 2 MEETINGS $15,570.00 <br />Task 3 OTHER SERVICES $5,190.00 <br />SUBTOTAL LABOR $80,352.80 <br />SUBTOTAL DIRECT COST $9,642.34 <br />TOTAL ESTIMATED COST $89,995.14 <br />Billings will be submitted monthly for the services completed during the prior month. <br />Subsequent to signing a contract agreement, the contents of this scope of work shall become a contractual <br />obligation if a contract ensues. Failure of a consultant to accept this obligation will result in the <br />cancellation of the contract award. Additionally, proof of insurance will be required prior to the City's <br />award of the contract. The City of Santa Ana requires liability insurance, workers compensation, <br />professional liability, and automobile insurance. <br />
