<br />f. Document significant task. <br />g. Ensure proper storage, handling and disposal of waste (i.e. manifest, profiles) with <br />disposal coordinator. <br />h. Ensure project area is left clean and operational. <br />i. Establish customer/regulatory satisfaction upon demobilization. <br />J. Ensure all appropriate paperwork is prepared with proper signatures of site contact prior <br />to departing site. <br />k. Communicate to ODC demobilization and significant times. <br />I. Responsible for the return of equipment and supplies in proper working order to the <br />district office. <br />m. Responsible for the return of all subcontracted equipment and materials, or for making <br />arrangements for their pick up. <br />14. The following protocol must be followed: <br />a. All equipment must be decontaminated on-site or immediately following the project at an <br />approved facility. <br />b. Cleaning of all trucks, trailers and equipment used on the project. <br />c. Damaged or out-of-service equipment must be tagged and communicated to the <br />Emergency Response Project Manager/Operations Coordinator for repairs/replacement. <br />d. All equipment re-stocked and made ready immediately for next call before termination of <br />job. <br />15. Collect report documentation with all applicable subcontracts and submit to project manager. <br />Documentation should include: <br />a. Health and Safety Plan <br />b. Job notes <br />c. Copy of project incident log. <br />d. Manifest <br />e. Photos <br />f. Incident narrative report. <br />g. Expense reports <br />h. Per Diem logs. <br />i. Disposal paperwork (authorization, generator information, etc.) <br />16. Immediately report any accidents or near misses to foreman, project manager and health and <br />safety manager. <br />17. Termination meeting to discuss project. <br />18. All project-related paperwork, notes and reports complete within one (1) business day oflast field <br />activity . <br />19. Time sheets are to be done daily. <br /> <br />Disposal Coordinator Responsibilities <br /> <br />1. IdentifY waste <br />2. IdentifY waste characteristics, liquid, solid, sludge, etc., and volume. <br />3. Communicate with customer requirements for waste disposal specific to customer needs. <br />4. Profile waste streams for facility acceptance. <br />5. Communicate to project manager/response foreman or customer the pricing for disposal and <br />transportation, and timing. <br />6. Facilitate manifesting, including customer signature and written approval for disposal options. Do <br />not sign manifest for customers (excluding TCEQ with proper language). <br />7. Provide follow-up documentation of acceptance or destruction of waste. <br />8. Forward all pertinent paperwork to project manager/response foreman. <br />9. Capture all true time for coordination of project on work tickets. <br />10. Log disposal transport, volume and pricing. <br />
