Continuous case management <br /> support will be provided <br /> Timeline: 12 months after exit <br /> <br />Partner/Collaborator Participation <br /> <br />To achieve the goal of serving more foster/emancipated youth, the Santa Ana Youth Council has <br />engaged community and faith base, education, government, judicial and social services agencies <br />to assist in this project. On April 27, 2005, the Superior Court of California, County of Orange, <br />issued a Judicial Order that allowed the sharing of information of youth in foster care. This <br />agreement has allowed pertinent information to flow between Family and Children Services, <br />Probation and WIA funded programs. <br /> <br />Linkages with WorJiforce Investment System <br /> <br />The Peer Mentors assigned to this project will participate in Santa Ana Youth Council activities <br />which include: monthly Youth Council meetings and YSPN meetings and the planning of the <br />annual youth forum and youth job fair. The Foster Youth Liaison Project will become a <br />participant of the Daisy Wheel ModelNSPN service delivery system. The Peer Mentors will <br />receive the same technical support and training from the Santa Ana WORK Center (One-Stop) <br />staff and WIB staff. <br /> <br />5 <br />
