Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />The transitional shelters offer supportive services, including individualized case management which aid the residents <br />in achieving goals in the areas of housing, health, finances, and life skills. These services will then help the client <br />understand the root causes of their homelessness and enable them to transition from a state of dependency to one of <br />self-sufficiency. Clients will meet with their program manager on an on-going basis. During case management, clients <br />will set and work to achieve their program-specific goals. Program Managers are then able to track the client's <br />progress and refer them to additional resources to address their needs. Additionally clients attend workshops and life <br />skills classes. The program staff also offer social events for the clients to bond with one another and create healthy <br />relationships. <br /> <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12- <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
