Number of ONLY unduplicated Santa Ana residents to be served by the funded program, named above, during the <br />12-month contract period, <br />I 351 Persons <br /> <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />The services that the Santa Ana Homeless Legal Outreach Program (SAHOP) provides include a full range of civil <br />legal services from advice and counsel to legal representation in matters involving, but not limited to, divorce, <br />paternity, child support, custody, domestic violence, landlord-tenant, SSI, and other government benefits and housing, <br /> <br />The purpose of SAHOP is to make these services more accessible to the homeless population by partnering with local <br />service providers that offer food, housing, psychological and employment counseling, and/or job training to individuals <br />and families who are homeless or are at risk of being homeless. Local partners are made aware of the legal services <br />provided through this grant and make referrals to Legal Aid through an expedited intake system, which provides for <br />immediate referral of clients who need legal assistance to LASOC. SAHOP staff regularly contacts homeless service <br />providers within our community and collaborates with providers that are interested in participating in this service <br />delivery system. <br /> <br />SAHOP strives to remove legal barriers, to provide wrap-around services to move these clients toward self-sufficieny, <br />and to increase access to legal services for the homeless by collaborating with all of the interested local agencies that <br />offer services to the homeless and by providing intake, case evaluation, advice and counsel and representation (when <br />appropriate) to 60 homeless clients at the Mental Health Association, the St Vicent De Paul Drop In Center, and the <br />Legal Aid Society. <br /> <br />Schedule of Pertormace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12- <br />month contract period per quarter.
