Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Only the <br />viewable space will print. <br />Under direction of the Outreach Program Manager, the Outreach Worker is responsible for providing outreach <br />services to homeless mentally ill adults with-in the Santa Ana area, with a focus on establishing linkages between the <br />homeless mentally ill and a variety of services including shelter, social services, medical and psychiatric care, and <br />MHA's Multi-Service Center as well as the continuum of services provided thoughout the County of Orange. The <br />Outreach Workers engage the Santa Ana homeless individuals in a variety of settings such as parks, food programs, <br />shelters, etc., to assess their need for services. They further assist homeless persons with transportation to medical <br />services, mental health clinics, shelter beels, social service agencies, and offer access to MHA's Multi-Service Center. <br />The Outreach Worker conducts initial assessments in the field to determine program eligibility and identify <br />entitlements and resources that the individual may require and for which they may be eligible. MHA maintains <br />statistics and other program paperwork in accordance with Agency (MHA), The City of Santa Ana and HUD <br />requirements. Staff attend regularly scheduled supervision and city/county meetings to futher enable them in providing <br />crisis intervention services to address acute psychiatric episodes, health emergencies, or personal crisis. The <br />Outreach Worker for Santa Ana is bi-lingual Spanish to assist clients with cultural, social and communication skills at <br />a level sufficient to engage consumers, to build trusting relationships, and to encourage them to participate in the <br />program. All MHA Staff have an excellent knowledge of entitlement programs, housing and shelter resources, legal <br />services, mental health and substance abuse services, health care resources, vocational training and rehabilitation, <br />and employment assistance. <br /> <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12- <br />ff they were served <br /> <br />month contract periOd per quarter.
