The case manager at the shelter is responsible for completing an <br />Intake with each client. Once the client has entered the shelter the a case manager meets weekly with the client to <br />compete the case management report (form 201) and the client self evaluation (form 206). These forms spell out what <br />the clients goals and needs are and help guide the resources that are provided to the client. These resources are <br />tracked using the resource referral form (form 302) and completed on a weekly basis. Additional information tracked on <br />this formm includes the specific time and date that the referral was given as well as the contact person and phone <br />number.
