Contract #: <br />Exhibit B <br />F. Individual Employment Plan: An Individual Employment Plan (IEP) shall be developed for <br />each customer enrolled into intensive services. This plan is a standardized document in the <br />Orange County Workforce Investment Area, and serves as the "road map" for services. Once <br />the customer is registered and a rationale for service levels is established, the IEP shall be <br />developed in partnership with the customer. Post-employment services shall be documented on <br />the IEP, including job retention strategies and other needed follow-up services. <br />G. Case Management: The Contractor shall provide integrated case management services to all <br />individuals that receive intensive and/or training services. Case management is considered a <br />key component of service delivery and critical to the ultimate success of the customers. Case <br />management is necessary to assure that all of the needs of the customers are met and that <br />information is collected and reported. If a customer is enrolled in more than one funding stream, <br />it is critical that differentiated services provided for each program are documented and justified. <br />H. File Maintenance and Documentation: A case file shall be maintained for every registered <br />customer. At a minimum, the case file will include information and documentation of each of the <br />following: <br />1. Program eligibility/determination of need; <br />2. WIA MIS forms, including source documents for validation; <br />3. Initial and Comprehensive Assessments; <br />4. IEP, including all updates of services provided and completed; <br />5. Approved ITA voucher if receiving training; <br />6. Progress reports, time and attendance if receiving WIA and non-WIA funded training <br />(including short-term pre-vocational training); <br />7. Counseling notes printed from the computer. <br />8. All components of the data validation tool referenced in WIA Bulletin 04-OCWDA 43. <br />I. Workshops: The One-Stop Center shall offer, at minimum, transferable skills workshops <br />weekly to registered WIA customers. These workshops are to help clients and case managers <br />determine the unique skills and strengths of this workforce. Transferrable skills workshops <br />should identify skills that can be applied to high-wage, high-growth demand occupations. The <br />intent of these workshops is to provide the knowledge and skills to identify potential jobs, apply <br />for a job, interview for a job and handle difficult situations that may arise after hire to ensure <br />successful job retention. <br />J. Training: The Contractor will be responsible for referring each individual to the most <br />appropriate activity for the individual as determined from the assessment and the documented <br />statements in the IEP. Not every customer will need or desire training. Training activities are for <br />those individuals who clearly cannot obtain or maintain employment without training in a <br />specific skill set or demand occupation. Successful completion of training courses must lead to <br />recognized credentials or their equivalent. The categories below list the training services that <br />shall be made available to individuals showing an assessed need. <br />1. Occupational skills training is offered through Individualized Training Accounts (ITA). ITAs <br />permit a wide range of choices to customers and are locally monitored to offer training in <br />only demand occupations in accordance with the Santa Ana WIB On-the-Job Training policy <br />2. On-the-Job Training (OJT) contracts will be developed under this agreement in accordance <br />with the Santa Ana WIB On-the-Job Training policy. <br />3. Contractor shall seek other non-WIA funded training, whenever possible, and shall use Pell <br />Grants to offset WIA funds where applicable. Other non-WIA funded training offered by the <br />community college, adult education and ROP is strongly encouraged. <br />P~g_e ~of f A <br />
