In <br />addition, self-assessment evaluations and qualitative interviews by staff determine progress. Annually an <br />assessment of the program's effectiveness indicated by strength of participant -reported feedback, community <br />support, quality of collaborative partnerships, and confidence of community and government social services to <br />offer referrals and form collaborations is also conducted. In addition, on-going systematic evaluation is <br />conducted by the leadership, staff and the volunteer Boardof Directors to identify corrective actions where <br />needed and to improve the program. <br />Exhibit A-1 <br />Page 1 of 1 <br />
