City of Santa Ana <br />Scope of Work <br />Name of Organization Women's Transitional Living Center, Inc. (WTLC) <br />Name of Funded Program In Pursuit of Wholeness <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded pro ram, named above, during the 12 -month contract period. <br />300 Persons <br />ll. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />The "In Pursuit of Wholeness" program is the mental health component of WTLC's services. This program is <br />designed to tager specific mental illness issues faced by homeless victims of domestic violence and their children. <br />These issues include: homelessness, violence/abuse, substance abuse, intergenerational effects that wreak <br />havoc with children and high -rick behaviors often seen in these children. <br />Objectives: <br />1) At least 300 people from the City of Santa ana will be identified as having diagnosabie mental illness problems. <br />Identification will be made through outreach into the community through ride-alongs with police, through the <br />Outreach Promotoras going into the community, through shelter clients, and through referrals from other agencies. <br />2) At least 300 people from Santa Ana will be provide the opportunity to enter into the mental health recovery <br />system. Referrals will be made for ALL the issues that a particular person faces: shelter/housing, substance abuse <br />treatment, food, clothing, counseling, prescriptions, counseling, legal/immigraion issues, child-care, etc. At least <br />250 people from Santa Ana will be provided additional services through mental health, WTLC, or another agency <br />providing mental health services. <br />3) At last 250 people from Santa Ana will become more stabilized through obtaining medication, acquiring ongoing <br />mental health services, acquiring shelter/housing/food/clothing, and/or stabilization of substance abuse issues. <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12 - <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
