SUBRECI R ENT agrees to provide CI TY with <br />written cumulative (year-to-date) reports of its activities on or <br />before the 15th day of October, January, April and July for the <br />period beginning July 1, and through and including the previous <br />three-month reporti ng peri od setti ng forth the acti vities, program <br />accompl ishments, new program i nformati on and yea -to -date <br />program statistics on expenditures, caseload and activities, When <br />appropri ate; pi ctures should be i nd uded. <br />(i i i) Quarterly ce tifi canon of match, pi us doc um ntati on of match <br />source. <br />(iv) Any such other reports as CI TY (or H U D) shat I reasonabl y <br />requi re and/or request, i nd udi ng but not I i mi ted to the fol I ow - <br />i ng i of ormati on: monthly reoords of ail I ethni c and rad al stati sti cs <br />of persons and fami I i es benefited by SUBRECI R ENT i n the per- <br />formance of its obligations under this Agreement, including, but <br />not I i mi ted to, the acti vi ties, program acconpl i shmen% new pro- <br />gram information, match fund certification and current program <br />stab sti cs on expenditures, caseload and activities. When appropri- <br />ate, pictures/sl ides should be i nd uded. <br />(3) RecordkeeQi ng of Ongoing M ai ntenance under the L BP Reps. I f the cervi ces and <br />program acts vi ti es provi ded by SU BRECI R ENT hereunder establ i sh an ongoi ng rel ati onshi p <br />with a property (such as long term tenant based rental assistance program [Subpart M] or transi- <br />ti ona l or permanent hoasi ng operated or admi ni stered by the SU BRECI R ENT), then the <br />SU B RECI R ENT shal I provi de a report to the CI TY evi denci ng that SU B RECI R ENT and/or the <br />property owner(s), as apps i cabl e, has performed ongoi ng mai ntenance of the subj ect property. <br />Ongoi ng mall ntenance shal I i nd ude evi dence of an annual d eaance certifi cati on of the subj ect <br />resi denti al property conducted by a DHS oertifi ed techni ci an to ensure that L BP hazard reduo- <br />ti on measures are mai ntai ned.
