A <br />program is developed mutually with the case manager and a weekly action plan is generated by the client which is <br />designed to take the client to the goal. The weekly progress is reviewed with the case manager and adjustments to <br />the plan are made as required to keep the client focused on the plan. <br />When ready to leave the shelter, assistance is given in finding a permanent residence and the client completes an <br />exit interview. The exit interview solicits the client comments for improving the program and if feasible the <br />suggestions are implemented. The goal is to have every client move to a permanent residence and independent <br />living. The success rate is typically 75%. <br />All records are written and are maintained in a client master file. Thomas House participates in HUD's HMIS(CMIS) <br />program and much of the data contained in the master client file is entered into the HMIS program.
