Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12 -month contract period. <br />120 Persons <br />II. Number of ONLY unduplicated Santa Ana residents to be served by the funded program, named above, during the <br />12 -month contract period. <br />39 Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />Joseph House, Regina House, and Emmanuel House all exist to end the homelessness of the residents who <br />enter into the program. Joseph House serves 20 single men, including veterans. Regina House serves single <br />mothers and their children and Emmanuel House serves individuals living with HIV/AIDS. The transitional <br />programs allow the residents to have a roof over their head while they receive individualized case management <br />and can eventually move into permanent housing. Upon entry into the program, clients meet with the program <br />manager to set goals in the areas of housing, health, finances, and life skills. It is a program requirement that <br />each resident meet with the program manager on an on-going basis in order to help the residents achieve their <br />goals. Program managers can track the positive changes which they see in the clients over time. Additionally, <br />clients must attend monthly workshops, life skills classes, and social events in order to improve their social skills. <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12 - <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
