Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded pro ram, named above, during the 12 -month contract period. <br />125 Persons <br />II. Number of ONLY unduplicated Santa Ana residents to be served by the funded program, named above, during the <br />12 -month contract period. <br />27 Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />Type in description here. <br />The CSP Youth Shelter is 12 -bed emergency crisis intervention shelter for homeless, runaway and at -risk youth <br />ages 11 to 17. The goal of the program is family reunification. The Shelter provides a "safe haven" for youth in <br />crisis. The services provided include, but are not limited to: individual, group and family counseling, tutoring, skill - <br />building activities, job/career readiness, independent living skills, physical exercise and recreation. Youth receive <br />shelter services for up to four weeks (based on their needs). The program is more than a refuge, as it aims to <br />provide long-lasting solutions to family problems. Aftercare services are also offered after youth exit the program. <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12 - <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
