The Cambodian Family FEZ Daisy Wheel Network Program Scope of Work July 1,2008 - June 30, 2009 I. TERM July I, 2008 through June 30, 2009 II. PURPOSE The FEZ Daisy Wheel Network Program will provide vocational/occupational training, employment strategies, education, and supportive services to individuals who reside in the Federal Empowerment Zone. III. POPULATION TO BE SERVED The population to be served are Federal Empowerment Zone residents 16 years and older. Eligibility will be determined by age, address, and by Right to Work documentation. IV. PROGRAM DESIGN, ELEMENTS, AND OUTCOMES FOR The Cambodian Family A. Program Design: I. Provide an intake assessment (for screening) of each FEZ resident that includes a review of academic and occupational skills levels as well as service needs; 2. Develop an Individual Service Plan including notifying achievement objectives. B. Program Elements: I. The following program elements will be made available to eligible residents expected to enroll in the FEZ Daisy Wheel Network Program for participation in activities aimed at providing a comprehensive strategy that addresses employment and training needs: i. vocational/occupational training, ii. employment strategies, iii. education, and iv. supportive services C. Performance Outcomes 1. 90 FEZ residents will be enrolled and served within 10 months. 2. Of the 90 FEZ residents served, 45 (50%) will be provided additional services within 10 months. 3. Ofthe 90 FEZ residents served, 27 (30%) will be placed in unsubsidized employment within 10 months. Cumulative Planned Enrollments (90 total: 70 adults, 20 youth): July - Sent. Oct. - Dee.
