In order to receive services <br />from LASOC, potential clients must first qualify to receive those services. LASOC currently utilizes an intake form <br />to qualify clients services. These services are provided on the spot and are entered into LASOC's Matrix <br />Database at a later time. The Matrix Database has the capability of creating detailed reports on the outcomes of <br />clients' cases in just a few minutes. This database is currently undergoing revisions that will enable automated <br />tracking of ESG activities, but at this time, client tracking must be conducted by the Matrix Database Designer. In <br />addition to the Matrix Database, clients' cases can be managed through paper case files, which are kept on <br />record for three years. For clients who have ongoing cases, the Supervising Attorney will utilize the Matrix <br />Database to continue to provide services until the case has been completed and services are no longer required. <br />Exhibit A-1 <br />Page 1 of 1 <br />
