<br />EXHmIT I <br />County of Orange Child Support Enforcement <br />Certification Requirements <br /> <br />In order to comply with child support enforcement requirements of the County of Orange, within 10 days of <br />award of Contract, the successful Contractor must furnish to the agency/department deputy purchasing agent: <br /> <br />A. In the case of an individual Contractor, hislher name, date of birth, Social Security number, and residence <br />address; <br /> <br />B. In the case of a Contractor doing business in a form other than as an individual, the name, date of binh, <br />Social Security number, and residence address of each individual who owns an interest of 10 percent or <br />more in the contracting entity; <br /> <br />C. A certification that the Contractor has fully complied with all applicable federal and state reporting <br />requirements regarding its employees; and <br /> <br />D. A certification that the Contractor has fully complied with all lawfully served Wage and Earnings <br />Assignment Orders and Notices of Assignment and will continue to so comply. <br /> <br />The certlflcadons will be stated as follows: <br /> <br />"I certify that is in full compliance with all applicable federal and state reporting <br />requirements regarding its employees and with all lawfully served Wage and Earnings A.ssignment Orders and <br />Natices af A.ssignments and will cantinue ta be in compliance thraughout the term of Contract <br />with the County of Orange. I understand that failure to comply shall constitllle a material breach of the cantracl <br />and thaI failure to cure such breach wilhin J 0 calendar days of nolice from the County shall canslitule grounds <br />for termination of the can tract. <br /> <br />It is expressly understood tbat this data will be transmitted to governmental _gucles cbarged witb the <br />establisbment and enforcement of cbild support orden and for no otber purposes and will be held <br />confideudalbythoseagencleL <br /> <br />Failure of the Contractor to timely submit tbe data and/or certifications required above or to comply witb <br />all federal and state reporting requIrements for cblld support enforcement or to comply witb all IawfuUy <br />served Wage and Earnings Assignment Orders and Notices of Assignment sball constitute a material <br />breach oClbe Coutract.
