One-on-one follow up; monthly contact to monitor, update, and revise 12- <br />month plan as needed; <br />2. One-on-one employment search and support; <br />3. One-on-one case management and support services <br />Describe how collaborations/partnerships will enhance pro4ram services and outcomes. <br />The OCCC is affiliated with the John Muir Charter School. OCCC corpsmembers who do not have <br />a high school diploma are enrolled in the OCCC on-site John Muir Charter School to acquire <br />credits and meet other requirements to earn their high school diploma. <br />In addition, the OCCC works closely with other community partners to provide a holistic <br />approach to youth development. For example, the OCCC collaboration with the Orange County <br />Probation Department provides a referral mechanism whereby youth transitioning from <br />incarceration can more easily access the services offered by the OCCC. Additionally, a member <br />of the Probation Department sits on the Board of the OCCC. <br />Furthermore, membership in the Youth Councils, such as the Santa Ana Youth Council, provides <br />an opportunity to network with other youth service providers and expand opportunities to <br />serve corpsmembers through cross-referrals. <br />Describe how the participant flow will occur. <br />Please refer to "Describe the overall services that will be mode available to the WIA participant" <br />(page 15) above and the "corpsmember Flow Chart" provided as an attachment to the <br />proposal. Both of these describe how participant flow will occur. Generally there are 4 phases <br />to OCCC's program and the corpsmember flow normally is: <br />• Work/school: corpsmembers spend the majority of their time working and going to <br />school. This may last from 3 months to 24 months depending on the number of high <br />school credits needed for graduation. <br />• Life Skills/Transition: Once corpsmembers earn their high school diploma or if they <br />have a diploma at the time of hire they enter the Life Skills and Transition phase of the <br />OCCC. corpsmembers normally spend 3-6 months completing a variety of activities <br />designed to help them become self sufficient, find employment and/or enroll in post- <br />secondary instruction. <br />• Program Completion: Once all program components have been completed, the <br />corpsmember has 60 days to exit the program into work and/or school with help and <br />support from staff. This time maybe extended based on individual needs. <br />• Follow-up: Minimum of 12 months follow up support from staff to remain employed <br />and stay in school; contact at least monthly if not more frequent to monitor and revise <br />12 month follow up plan; <br />Not all corpsmembers complete all four phases as some choose to leave for a job or school and <br />enter the follow-up phase as soon as they earn their high school diploma. All corpsmembers, <br />regardless of their reason for leaving the OCCC, are included in the 12 month follow up phase <br />and have access to OCCC staff and resources (computer lab, fax, voicemail) for maintaining <br />Page 17 of 24 <br />
