Team building and other crew/peer centered activities to develop responsibility <br />and positive social interactions. <br />EDUCATION <br />1. Individual education goals (high school diploma and/or post-secondary <br />instruction) <br />2. Study skills; tutoring; secondary school instruction; dropout prevention <br />strategies; <br />3. Basic skills remediation; <br />4. Literacy and numeracy gains; <br />5. CA High School Exit Exam; tutoring, preparation and administration; <br />6. OCCC Charter School attendance & participation; earn high school diploma; <br />7. Field-based learning activities to incorporate field work projects with the <br />classroom; <br />8. Quarterly recognitions, field trips, and other activities to build positive social <br />interactions. <br />INDIVIDUALIZED CASE MANAGEMENT & SUPPORT SERVICES <br />1. Referrals for comprehensive guidance and counseling for legal, medical, <br />substance abuse, family/personal, health, housing, etc. issues <br />ADULT MENTORING (occurs throughout program as staff works with corpsmembers in <br />various capacities) <br />TRANSITION SERVICES: 10-13 weeks based on individual needs <br />1. Pre-employment skills training <br />2. Individualized job search and placement <br />3. Individualized support/assistance for continuing post-secondary education; <br />financial aide packets, enrolling on-line, etc. <br />4. Individualized case management and support services <br />5. Adult mentoring <br />6. Life skills <br />PROGRAM COMPLETION: <br />60-day timeline for post-corps placement; Maybe extended based on individual <br />needs or circumstances <br />POST CORPS TRACKING: Unspecified time, available as long as former corpsmembers seek <br />help and support, minimum of 12 months <br />Page 16 of 24 <br />
