The TAG also suggests using <br />verification document(s) that satisfy List A or that satisfy List B and C of the 1-9 <br />form. Once a Case Manager determines that an applicant is no less than 17 and <br />no more than 21, documentation is collected to determine economic eligibility. <br />Using the TAG, a Case Manager will check to see if the applicant is low-income. <br />Among the criteria for Iow-income are Cash Public Assistance, Family Income <br />(total means of support for family for a period of six months), Foster Child, <br />Homeless, or an Individual with a Disability. The TAG outlines documents that <br />are acceptable for WIA eligibility. In the case of Family Income, documentation <br />is provided for each applicable inclusive income source received by the applicant <br />and each family member for the six-month income period immediately preceding <br />the determination date. It is also necessary to verify family size when utilizing <br />family income eligibility. Again, only TAG approved documents are collected. <br />Intake / Assessment: Project RYTMO has established a method for assessing <br />each student and determining appropriate services. After being accepted into the <br />program, every student is assigned a case manager. The case manager will work <br />with the student to develop an lindal vidual Service Plan (ISP) — an assessment tool <br />that will be used to set individual program and skill development goals. The ISP <br />is used to evaluate the student's progress for the duration of the program and must <br />be updated weekly. Prior to the development of the ISP, a written "pre-test" is <br />administered to students to determine competency and knowledge of the given <br />subject matter. A comprehensive "post-test" is administered to students at the end <br />of the 8 -week program. The result provides a simple and effective method of <br />evaluating progress, knowledge retention and general overall progress. <br />Case Management: Subscribing to a "holistic" approach, Project RYTMO staff <br />uses case management to erase the barriers to success that face our participants. <br />Holistic case management is working with all aspects of the participant: physical, <br />social, mental, and developmental. Case management, specifically, includes: <br />counseling, developing a career goal, tutoring, mentoring, and advocacy - <br />anything that assists the participant in achieving self-sufficiency. Project <br />RYTMO staff uses a variety of resources and referrals to aid in the case <br />management process. It is truly a team effort. <br />Case management is recorded through case notes, the Individual Service Plan <br />(ISP), and MIS paperwork. Case Managers determine enrollment dates based on <br />the eligibility approval date. Consequently, it is at that time that services begin. <br />Moreover, case managers also provide student/parent orientation, personal and <br />career assessment, as well as matching them with a compatible adult mentor. <br />Program Services / Parent Involvement: Project R.Y.T.M.O. encourages <br />youth to express their creativity through the vehicle of music.
